Completing a Super Choice Form
Not sure what to write on your Super Choice Form? This guide tells you where to get the information you need and which sections to fill in.
In Section A on the first page, you need to complete sections 1, 2 and 3.
Section 1: Most people will have an APRA or RSA.
Section 3: For your member number, check your super statement or contact your super fund directly for your member number.
For required documentation, your superfund will have a free downloadable compliance letter on their website that you can give to your employer with this form, the letter will also include their address, ABN and USI to help you complete this form.
On the second page, you need to complete section 5 by checking the box, and adding your signature and the date.
If you are a contractor and eligible for super from your employer, you will also need to give them your date of birth so they can process your super contribution.
